Current or Former Pastor, First Lady, Pastor's Wife or Community Leader Must demonstrate a commitment to the work of the church, the health and wellbeing of her family, and community enrichment Church or community affiliation must be within 100 miles of Louisville, KY

Section 5 - Please use the space below, to tell us about your nominee and why she deserves to be a First Lady of Louisville

+ You may nominate as many as you want but you cannot nominate yourself.
+ Your nominee must accept the nomination and can withdraw her acceptance at any time.
+ If she chooses to withdraw, her nomination is forfeited for the current year.
+ If your nominee is selected, we will ask for your assistance in contacting her. If we are not able to make contact, we will have to withdraw her nomination.
+ Upon request, we will make every effort to keep your nomination. We will need proper contact information for your nominee to ensure your anonymity.
